When you should use this form

You should only use this form once you have booked your ceremony.

You do not need to complete this form if your ceremony is taking place in the Register Office or if you are having a ‘Bijou’ ceremony in the Floral Room.

You should only complete and submit this form to us once you have decided on your ceremony content – this includes readings, personal vows and music. We are not permitted to include any religious content.

If you are including personal vows and wish to keep them as a surprise for your partner, you can email them separately to us. Include details of your ceremony date and time.

You need to submit this form at least 2 weeks in advance of your ceremony date.

Use this form to let us know the details of your marriage or civil partnership ceremony.

You can tell us:

• your name and contact details
• the names of your witnesses
• other special people involved in your ceremony
• how you prefer to make your entrance
• your choice of declarations or promises
• your choice of music during your ceremony
• if you are using rings, how these will be presented
• any readings
• any other details you would like to include
